Signs & Symptoms of Pregnancy

๐Ÿคฐ Signs & Symptoms of Pregnancy

Definition: Pregnancy is the condition in which a fertilized ovum develops inside the uterus from conception to birth. Recognizing its signs and differentiating them from other conditions ensures early and accurate diagnosis.

Classification

The signs of pregnancy are grouped according to diagnostic reliability:

  1. Presumptive (Subjective) — felt by the woman
  2. Probable (Objective) — observed by the examiner
  3. Positive (Diagnostic) — confirmed by fetal evidence

1️⃣ Presumptive Signs (Subjective)

Experienced only by the woman and therefore least reliable, since many other conditions can mimic them.

Sign Description Mimicking Conditions
Amenorrhea Absence of menstruation due to implantation & hormonal change Stress, endocrine disorders, malnutrition, menopause
Nausea & vomiting Morning sickness from ↑ hCG and estrogen Gastritis, GI infection, anxiety
Breast changes Tenderness, fullness, darkened areola PMS, hormonal therapy, mastitis
Fatigue Increased tiredness from metabolic demand Anaemia, infection, hypothyroidism
Frequent urination Pressure of enlarging uterus on bladder UTI, caffeine use, anxiety
Quickening First feeling of fetal movement (16-20 wks) Intestinal gas, muscle twitching

2️⃣ Probable Signs (Objective)

Observed by the examiner and more reliable, but still not conclusive because certain disorders can mimic them.

Sign Description Mimicking Conditions
Abdominal enlargement Growing uterus Fibroid, ovarian cyst, obesity, ascites
Goodell’s sign Softening of cervix (6-8 wks) Cervicitis, hormonal therapy
Chadwick’s sign Bluish discoloration of cervix/vagina Pelvic congestion, infection
Hegar’s sign Softening of lower uterine segment Uterine tumor, pelvic congestion
Ballottement Rebound of fetus when cervix tapped Ovarian cyst, uterine mass
Positive hCG test Detection of pregnancy hormone Choriocarcinoma, molar pregnancy, drug interference

3️⃣ Positive Signs (Diagnostic)

These confirm the presence of a living fetus and are conclusive of pregnancy.

  • Fetal heartbeat: Heard by Doppler (10-12 wks) or fetoscope (18-20 wks).
  • Fetal movements: Palpated by examiner after about 20 wks.
  • Ultrasound visualization: Gestational sac visible by 5-6 wks.

๐Ÿงพ Summary

Category Nature Examples Reliability
Presumptive Felt by woman Amenorrhea, nausea, breast changes Least reliable
Probable Observed by examiner Goodell’s, Chadwick’s, hCG test Suggestive
Positive Due to fetus Fetal heartbeat, movement, ultrasound Conclusive
๐Ÿ’ก Key Takeaway:
Presumptive = Felt | Probable = Observed | Positive = Confirmed.
Always verify findings with urine hCG and ultrasound before diagnosis.
